(New Patient/Initial Visit) - 2 Dental Cleanings

- 50% Full Mouth Series X-Rays & Panoramic X-Rays

- Single Adult – $385
- 2 Adults – $770
- Family of 3 (2 adults, 1 child) – $1,100
- Family of 4 (2 adults, 2 children) – $1,430

Membership Guidelines
Membership fees are due the day of joining the program
No refunds of premiums will be issued at any time
Children listed on plan must live in household or be a full time student
Cosmetic procedures such as teeth whitening and Invisalign® are not included in the plan discount
Membership expires one year from joining the program
